If you are more than 10 minutes late to your appointment, it will be considered a missed appointment and you will need to reschedule. Missed appointments that are not cancelled in advance will incur a $25 charge. New patients that do not show to their first appointment will not be rescheduled. We appreciate 24 hours notice if cancellation or rescheduling is needed.

48 business hours are required for medical record requests.


We accept several forms of payment:

– Checks (made out to Upstream Family Medicine)

Upstream Family Medicine is a preferred provider with Aetna, Blue Cross, Cigna, and Moda.

Medicaid Patients, please be advised we do NOT provide travel vouchers.

Under some circumstances, we are able to bill other insurance providers. Please call for more information.

We are not accepting new Medicare patients at this time.
