Please reach out to schedule or make changes to an appointment.

LATE, MISSED, OR CANCELED
APPOINTMENT POLICY
Out of respect to our scheduled patients,
if you are more than 10 minutes late to your check-in time, we will need to reschedule your appointment. This is considered a missed appointment. Missed appointments not canceled 24 hours in advance will incur a $25 charge.

New patients that do not show to their first appointment will not be rescheduled.

 

Payments

We accept several forms of payment:

– Cash
– Checks (made out to Upstream Family Medicine)

 

Payments can be made by:

– Portal
– Phone
– Mail

Insurance

Upstream Family Medicine is a preferred provider with Aetna, Blue Cross, Cigna, and Moda.

Medicaid Patients, please be advised we do NOT provide travel vouchers.

Under some circumstances, we are able to bill other insurance providers. Please call for more information.

We are not accepting new Medicare patients at this time.
